Participant Waiver & Release of Liability

1. ASSUMPTION OF RISK

I acknowledge that participation in hockey training, camps, and related activities involves inherent risks, including but not limited to:

  • Falls, collisions, and contact with other participants
  • Injury from pucks, sticks, boards, or ice surfaces
  • Physical exertion and strain
  • Equipment-related injuries

I understand that these risks may result in serious injury, permanent disability, or death.

I voluntarily accept and assume all risks associated with participation in Unit III programs.

2. RELEASE OF LIABILITY

In consideration of participation, I hereby release, waive, and discharge:
Unit III Hockey, Kevin Labbé, all instructors, staff, contractors, and facility operators
from any and all liability for injury, loss, or damage arising from participation, including negligence.

This release applies to:

  • Personal injury
  • Property damage
  • Any claims arising during or after participation

3. MEDICAL CONSENT

I certify that the participant is physically fit to participate in hockey activities.

In the event of injury or medical emergency, I authorize Unit III staff to:

  • Provide basic first aid
  • Seek medical treatment if necessary
  • Contact emergency services

I understand that I am responsible for any medical costs incurred.

4. CONCUSSION ACKNOWLEDGEMENT

I acknowledge that participation in hockey activities carries a risk of concussion.

I understand the importance of:

  • Recognizing the signs and symptoms of concussion
  • Reporting any suspected concussion immediately
  • Removing the participant from activity if a concussion is suspected
  • Seeking proper medical evaluation before returning to play

I agree that the participant will not return to activity until medically cleared if a concussion is suspected or diagnosed.

5. CODE OF CONDUCT

The participant agrees to:

  • Follow all instructions from coaches and staff
  • Respect other players, staff, and facilities
  • Participate in a safe and controlled manner

Unit III reserves the right to remove any participant for unsafe or inappropriate behaviour without refund.

6. MEDIA RELEASE

I grant permission for Unit III to use:

  • Photos
  • Video footage
  • Training content

featuring the participant for:

  • Social media
  • Marketing
  • Promotional materials

No personal identifying information will be shared without consent.

7. CANCELLATION & REFUND POLICY

Registrations are eligible for a full refund only if cancellation is made more than 72 hours prior to the scheduled start time.

Cancellations made within 72 hours of the scheduled start time are non-refundable.

No refunds will be issued for:

  • Missed sessions
  • Partial attendance
  • Removal due to conduct

8. ACKNOWLEDGEMENT

I confirm that:

  • I have read and understand this agreement
  • I understand the risks involved
  • I agree to all terms voluntarily
